Metatarsalgia: understanding why the forefoot is overloaded

Metatarsalgia often presents as pain, pressure or discomfort in the front of the foot. To manage it well, we need to understand where load concentrates and what keeps it there.

The painful area does not always tell the whole story

Metatarsalgia describes discomfort in the front of the foot around the metatarsal heads. It may appear when walking, standing, wearing particular shoes or during sport. Some people describe burning, a pebble-like sensation, localised pressure or forefoot fatigue.

At MEDIPIE we assess how load reaches that area and why it may persist. We consider loading, mobility, footwear, activity, the space available inside the shoe and the patient's tolerance. Offloading needs to be precise: an orthosis that is too thick, too rigid or poorly fitted may not work in real life.

  • Mobility of the first ray, toes and ankle.
  • Work, sport and time spent standing.

How we plan forefoot offloading

When appropriate, an orthosis may help redistribute pressure and unload specific forefoot areas. The key is choosing materials and geometry that work with the patient's footwear.

Not every metatarsalgia is managed in the same way. If another lesion, entrapment, joint problem or medical condition is suspected, we explain when further assessment is needed.

More than one painful area

The type of symptom changes the assessment

Localised pain under one metatarsal head, diffuse pressure, burning, tingling, joint swelling or pain after trauma do not point to the same problem. We also review callus, toe deformity, mobility, sensation and the relationship between the forefoot and footwear.

Metatarsalgia describes a painful region; examination needs to identify which structure and mechanism may be involved before an offloading strategy is chosen.

A usable plan

Offload without creating a new problem

An offloading element that is misplaced, too high or incompatible with the shoe may increase pressure or cause the patient to stop using it. We therefore test volume, position and tolerance inside usual footwear.

If there is marked swelling, acute pain after injury, loss of sensation, colour change or an atypical course, the assessment is widened before treatment is considered.

Evidence and limits

Reducing plantar pressure does not mean there is one correct orthosis

A 2024 systematic review and meta-analysis found that bespoke orthoses could reduce pressure under the central metatarsal heads compared with no treatment, but pressure outcomes were similar to standard footwear, standard orthoses or metatarsal domes in the available studies.

This supports an important point for patients: offloading may be useful, but it does not show that a custom orthosis is always superior or that every metatarsalgia should be treated with one. The decision depends on diagnosis, clinical objective, footwear and tolerance.

Common questions

Is metatarsalgia one specific diagnosis?

Not exactly. Metatarsalgia describes pain in the front of the foot, but it may relate to overload, mobility, metatarsal shape, joints, nerves, soft tissues or footwear. Identifying the pattern helps decide which type of offloading may make sense.

Can an orthosis unload the forefoot?

It may help when clinically indicated. Material selection, geometry and accurately positioned offloading can redistribute pressure at the forefoot, but not every metatarsalgia needs an orthosis or a custom device. Position, volume and tolerance need to be checked with the real foot and footwear.
