At MEDIPIE we assess, design and fabricate each orthosis and fit it directly to the foot using materials selected for the individual case.

Personalisation starts by defining the function

A foot orthosis is not personalised simply because it reproduces the shape of the foot. First it needs a specific objective: unloading, controlling, stabilising, facilitating or compensating for something identified during assessment.

From that function we design the geometry, stiffness and combination of materials. Direct fitting and follow-up then allow us to see how the treatment performs in real life.

Individual indication

Not every variation in foot function needs orthoses. A footprint is only one data point: indication depends on symptoms, function, gait, activity, footwear and the clinical objective of the individual case.

In-house fabrication

We design and fabricate each orthosis at MEDIPIE. Resins, materials, stiffness, thickness and reinforcements are selected according to the assessment and clinical objective rather than sending treatment to an external laboratory.

Fitting and follow-up

We shape the orthosis directly to the foot and check it in the patient's real footwear. We then review comfort, response and wear so adjustments can be made when needed.

From assessment to the foot, with control of the whole process

The clinical objective determines the design, stiffness, thickness and combination of materials. The orthosis is fabricated in our workshop and adapted directly to the foot so that its shape and intended function can be checked within the same process.

We then assess footwear space, initial comfort and tolerance. Follow-up allows the treatment to be adjusted if daily use, footwear or the patient's response changes.

An orthosis may aim to unload, stabilise, improve comfort or support function; not every device aims to make the same change and it does not replace other assessments when they are needed.

Individual design

What can differ between two personalised orthoses

Similar feet or footprints do not require identical treatment. Personalisation lies in the clinical and technical decisions made for each use.

Geometry and stiffness

Base shape, flexibility and reinforcement are selected according to the objective, mobility and expected tolerance.

Relief and materials

Support, relief, cushioning and finishing are positioned according to sensitive areas and the intended function.

Volume and footwear

Thickness and contour have to work with the actual shoe; a technically sound solution that cannot be worn is not a good solution.

Evidence depends on indication

An orthosis does not have the same role in every problem

Evidence for foot orthoses depends on the diagnosis, objective and the intervention they are compared with. We therefore do not use the label “custom orthosis” as a guarantee of outcome or proof of superiority by itself.

An orthosis may be part of a plan when indicated, but is not presented as a universal stand-alone treatment.

What personalisation means

In-house fabrication provides clinical traceability; it does not prove automatic superiority

Designing, fabricating, fitting and reviewing the orthosis within the same MEDIPIE process maintains continuity between assessment, objective and later adjustments. That continuity is a feature of our method.

We do not use it to claim that a custom orthosis is better for every diagnosis. Indication and expected outcome depend on the specific problem, footwear, activity and individual response.
